Position Description

JOB TITLE: Clinical Coordinator Bariatric

DEPARTMENT: MH SURGICAL CARE

EXEMPT FROM OVERTIME: Yes

JOB SUMMARY
Is responsible for the development and implementation of the Bariatric program needs and ensures compliance with MBSAQIP standards, educates patients and hospital staff, and ensures development and implementation of initiatives to positively impact quality patient care. As the coordinator of care, treats all internal and external physicians, staff and patients/families as valued customers striving to provide advocacy and service. Work directly with the Bariatric and General Surgeon(s). Works in collaboration with the Bariatric Medical Director to manage the care coordination of the Bariatric Program for metabolic and bariatric surgery patient population.

MINIMUM QUALIFICATIONS
Education:
• Bachelor's Degree (Required)
• Master's Degree

Experience/Physical Requirements:
Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs. Minimum of three years clinical experience in nursing or registered dietician experience.

Licensure/Certification:

• Registered Nurse (Required)
• Certified Bariatric Nurse (Required)

JOB SPECIFIC RESPONSIBILITIES
Excellence comprehensive surgical/bariatric care Facilitates scheduling of appropriate outpatient and inpatient visits as required to ensure timely access to medical care. Develops system to ensure patient scheduling for appropriate imaging (MRI, CT, etc.) and/or interdisciplinary assessments and interventions are done in as few visits as possible. Assures all preoperative requirements/tests are completed and approved prior to surgery. Coordinates patient transfer between services & disciplines. Serves as a liaison between hospital and surgical practice. Oversees Bariatric clinical guidelines, monitors, tracks, and trends patient outcomes. Assists in the development and revision of policies, procedures, and clinical guidelines. Provides direct patient care as applicable to facilitate the plan of care. Building and maintaining positive team, patient, Physician, and public relationships Maintains written/verbal communication with referring institutions, physicians, and insurance companies as indicated. Teaches patient and family pre and post operative classes and evaluates the effectiveness of teaching efforts and modifies plans, as needed. Facilitates development, implementation, and promotion of strategies for the delivery of patient care and achievement of positive patient outcomes in a teaching environment. Meets with surgeons, nursing, and other disciplines on a regular basis to address operational issues. Provides professional in-services, and education as necessary for staff and patients in both inpatient and outpatient settings. Provides public in-services, as requested. Coordination of multidisciplinary plans of care Ensures the medical plan is carried out by evaluating patient response to interventions. Communicates the plan of care to other departments and health care providers to support optimal patient outcomes. Ensures effective communication and collaboration among the health care team members, addresses inter and intra-departmental issues. Intervenes in the coordination of ancillary testing and follow-up visits as necessary. Quality Improvement/Compliance Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings. Consistently follows established standards of care, standards of practice and policies and procedures. Collects data for quality improvement activities and assists in developing and analyzing data. Develops corrective action plans as appropriate. Participates in research that may improve patient care. Ensures appropriate evaluation of satisfaction (patient, family, referring physician, etc.). Analyzes, trends, and utilizes data to improve patient care and outcomes to develop the bariatric program. Maintains oversight of ongoing Bariatric Service Center of Excellence (BSCOE) compliance.
